Responsible for providing skilled physical therapy interventions to patients following established orders and goals, and based on standards of practive. The PTA works under the ongoing supervision of the Physical Therapist. This position reports to the Clinical Manager.
The Clinical Liaison serves as an advocate for patients in the pre-admission process to Home Care and continuing until the patient is discharged from the facility prior to admission to HomeCare Maryland. Liaisons work with patients, families as well as referral sources to coordinate transitions to home.
To perform initial and ongoing patient assessments. To perform skilled occupational therapy functions. To provide supervision and direction to the certified Occupational Therapy Assistant (COTA) providing care in the certified home care. To act as case manager for occupational therapy only cases.

Responsible for providing professional nursing care for clinical patients following established standards and practices. Functions as primary clinical liaison between patients and physicians. This position also reports to Service Director for compliance, administrative and operational duties.
