Community Health Nurse Practitioner
Position Summary: Provide home-based care to post-acute patients from all LifeBridge Health owned or joint venture facilities that require health care in the home setting. Performs patient education, coordination of services and referrals to community resources if required. Under general supervision, performs comprehensive medical and nursing evaluation to diagnose and treat patients. Proceeds alone on regular duties, referring questionable cases to medical supervisor. Advocate for patients to receive community, social and health care services. Collaborate with other health care providers regularly to ensure that patients' physical, mental, functional and psychosocial needs are met. Provide support for family members of sick and ill patients. Determine whether a home setting is the best place for a patient to receive care. Take a leadership role in advocating for changes in health care policies related to health care delivery, quality and reimbursement. Functions as an Advanced Practice Nurse.
Qualifications/Requirements: Master's degree in an advanced practice nurse practitioner program with Acute Care Certification. Must be a graduate of an accredited school of nursing attaining a BSN degree. Recent nurse practitioner experience 2-3 years. Maryland Registered Nurse License BLS/ACLS Acute Care NP certification Fleet safety Valid driver's license required.
