Occupational Therapist

Essential Functions:

• Conducts initial and ongoing assessments of the patient’s clinical and functional limitations, environmental safety, and adaptive equipment use

• Determines need for referral to additional services

• Conducts the recertification/resumption of care comprehensive assessment, with OASIS data items and medication review

• Develops the Plan of Care for therapy interventions based on assessment findings which include ADL and IADL functioning level

• Develops realistic, individualized patient therapy goals

• Conducts ongoing review of the Plan of Care and makes revisions as patient condition warrants

• Accurately documents interventions and findings from each home visit in timeframe required by agency policy

• Development and evaluation of the plan of care in partnership with the patient, representative (if any) and caregivers.

• Providing services that are ordered by the physician as indicated on the plan of care

• Prepare complete and accurate clinical notes

• Communications with all physicians involved in the plan of care and other health care practitioners

• Participate in the QAPI program

• Participation in sponsored in-service training



• Occupational Therapy license for the state of Maryland

• Prior experience in home care preferred, but not required



• 2-3 years experience as an Occupational Therapist


Certifications and License:

• Current Maryland state Occupational Therapist License

• Current American Heart Association BLS Healthcare provider certification

• Valid driver’s license and automobile insurance
